Abstract
Background and Objectives: Fuel storage tanks are among the major hazard sources in power plants, posing significant risks to personnel, equipment, and the environment. Therefore, identifying and assessing the associated safety and environmental risks, together with implementing appropriate control measures to reduce the likelihood and consequences of accidents, is of paramount importance.
Materials and Methods: In this study, fuel leakage from diesel storage tanks was analyzed as a critical top event in power plants. Initially, the most significant causes leading to the occurrence of the top event were identified and evaluated using the Fault Tree Analysis method. Subsequently, the potential consequences resulting from the occurrence of the top event were predicted through the application of the Event Tree Analysis technique. Moreover, the most important control measures aimed at preventing fuel leakage and mitigating its consequences were identified and recommended according to their priority.
Results: Overall, five principal threats, together with 87 intermediate events, were identified as contributing factors to fuel leakage from diesel storage tanks in power plants. In addition, 49 control measures aimed at preventing the occurrence of fuel leakage were identified and proposed.
Conclusion: Based on the analysis of the results and the constructed diagrams, it was determined that the most important control measures for preventing fuel leakage from diesel storage tanks, in order of priority, include periodic inspection and maintenance, the development of safety procedures, and the provision of relevant training programs.
