<p dir="RTL"><strong>مقدمه: </strong>خطاهای انسانی علت بسیاری از حوادث اعم از صنعتی و پزشکی می<span dir="LTR"></span>باشند، به همین دلیل پیدا کردن رهیافت<span dir="LTR"></span>هایی برای شناسایی و کاهش آنها بسیار حائز اهمیت است. از آنجائیکه تاکنون در زمینه خطاهای دندانپزشکی مطالعات اندکی انجام شده است، لذا تصمیم بر آن شد تحقیقی با هدف شناسایی و ارزیابی خطاهای انسانی در دستیاران تخصصی اندودونتیکس دانشکده دندانپزشکی دانشگاه علوم پزشکی کرمان با استفاده از تکنیک <span dir="LTR">SHERPA</span> انجام شود.</p>
<p dir="RTL"><strong>روش</strong> <strong>کار:</strong> این تحقیق کیفی و بصورت مقطعی در سال 1393 انجام شد. اطلاعات با استفاده از مشاهده مستقیم فعالیت<span dir="LTR"></span>ها و مصاحبه با دستیاران تخصصی اندودونتیکس جمع آوری گردید. در مجموع 10 وظیفه بحرانی که در آنها احتمال ایجاد آسیب به بیماران زیاد بود، شناسایی شدند. پس از تجزیه و تحلیل سلسه مراتبی وظایف (<span dir="LTR">HTA</span>)، شناسایی خطاها به روش <span dir="LTR">SHERPA</span> انجام و برگه<span dir="LTR"></span>های کار مربوطه تکمیل گردید.</p>
<p dir="RTL"><strong>یافته</strong><strong><span dir="LTR"></span></strong><strong>ها:</strong> با تجزیه و تحلیل برگه<span dir="LTR"></span>های کار <span dir="LTR">SHERPA</span>، 90 خطا شناسایی شد که از این تعداد، 7/67 % خطاها از نوع عملکردی، 3/ % خطاها از نوع بازدید، 8/8 % خطاها از نوع انتخابی، 5/5 % خطاها از نوع بازیابی، 4/4 % خطاها از نوع ارتباطی بودند. در نتیجه بیشترین نوع خطاها از نوع عملکردی، کمترین آنها از نوع ارتباطی بود.</p>
<p><strong><span dir="RTL">نتیجه</span></strong><strong></strong><strong><span dir="RTL">گیری: </span></strong><span dir="RTL">نتایج حاصل از این تحقیق نشان داد که بیشترین درصد فراوانی خطاها و همچنین بیشترین درصد سطح ریسک مربوط به خطاهای عملکردی با سطح ریسک نامطلوب می</span><span dir="RTL">باشد، لذا برای کاهش وقوع این قبیل خطاها و محدود کردن پیامدهای آنها، لازم است راهکارهای کنترلی از جمله آموزش</span><span dir="RTL">های ادواری روش صحیح انجام کار، تهیه چک لیست</span><span dir="RTL">های نظارتی، تدوین دستورالعمل و ایجاد یک سیستم گزارش</span><span dir="RTL">دهی منظم و استاندارد، عملیاتی ارائه گردد. با توجه به نتایج بدست آمده از این مطالعه، کنترل خطاهای بازیابی که بیشترین درصد خطاهای با سطح ریسک نامطلوب، و خطاهای عملکردی که بیشترین درصد فراوانی کل خطاها را به خود اختصاص دادند باید در اولویت قرار گیرند.</span></p>
<p><strong>Introduction</strong>: Human errors are the cause of many accidents, including industrial and medical, therefore finding out an approach for identifying and reducing them is very important. Since no study has been done about human errors in the dental field, this study aimed to identify and assess human errors in postgraduate endodontic students of Kerman University of Medical Sciences by using the SHERPA Method<span dir="RTL">.</span></p>
<p><strong>Methods</strong><strong><span dir="RTL">:</span></strong> This cross-sectional study was performed during year 2014. Data was collected using task observation and interviewing postgraduate endodontic students. Overall, 10 critical tasks, which were most likely to cause harm to patients were determined. Next, Hierarchical Task Analysis (HTA) was conducted and human errors in each task were identified by the Systematic Human Error Reduction Prediction Approach (SHERPA) technique worksheets<span dir="RTL">.</span></p>
<p><strong>Results</strong><strong><span dir="RTL">:</span></strong> After analyzing the SHERPA worksheets, 90 human errors were identified including (67.7%) action errors, (13.3%) checking errors, (8.8%) selection errors, (5.5%) retrieval errors and (4.4%) communication errors. As a result, most of them were action errors and less of them were communication errors<span dir="RTL">.</span></p>
<p><strong>Conclusions</strong><strong><span dir="RTL">:</span></strong> The results of the study showed that the highest percentage of errors and the highest level of risk were associated with action errors, therefore, to reduce the occurrence of such errors and limit their consequences, control measures including periodical training of work procedures, providing work check-lists, development of guidelines and establishment of a systematic and standardized reporting system, should be put in place. Regarding the results of this study<span dir="RTL">, </span>the control of recovery errors with the highest percentage of undesirable risk and action errors with the highest frequency of errors should be in the priority of control<span dir="RTL">.</span></p>
