fa بررسی روند تغییرات هورمون تیروئید کارگران صنعت کاشی و سرامیک در ارتباط با مواجهه مزمن با صدا: یک مطالعه کوهورت 5 ساله Investigation of Thyroid Hormone Trends in Ceramic and Tile Industry Workers in Relation to Chronic Noise Exposure: A 5-Year Cohort Study عوامل فیزیکی Physical agents مقاله پژوهشي Research Article زمینه و هدف: آلودگی صوتی از مهم‌ترین عوامل زیان‌آور محیط کار است. یکی از پیامدهای احتمالی مواجهه طولانی‌مدت با صدا، تغییر در عملکرد غدد درون‌ریز به‌ویژه هورمون‌های تیروئیدی است. این مطالعه با هدف بررسی روند تغییرات هورمون TSH در کارگران صنعت کاشی و سرامیک طی پنج سال و ارتباط آن با مواجهه مزمن با صدا انجام شد. روش بررسی: این پژوهش یک مطالعه کوهورت تاریخی است که در سال‌های 1395 تا 1400 در یکی از صنایع کاشی و سرامیک انجام شد. تعداد 100 نفر از کارگران بخش‌های پرصدا به عنوان گروه مواجهه و 100 نفر از کارگران بخش‌های کم‌صدا یا اداری به عنوان گروه کنترل در نظر گرفته شدند. اطلاعات دموگرافیک و مقادیر هورمون TSH از پرونده‌های پزشکی استخراج و ثبت شد. داده‌ها با آزمون تی مستقل و تحلیل واریانس برای اندازه‌های تکراری در نرم‌افزار  SPSS 24تحلیل گردید. یافته‌ها: میانگین سطح TSH در سال پایه (1395) در گروه مواجهه و کنترل به ترتیب 53/1±93/2 و 36/1±89/2 بود. طی سال‌های مطالعه، تغییرات این هورمون در هر دو گروه روندی نامنظم داشت و در سال 1400 به ترتیب به 59/1±14/3 و 35/1±10/3 رسید. نتایج آزمون تی مستقل و تحلیل واریانس نشان داد که تفاوت معناداری در تغییرات TSH بین دو گروه در طول زمان وجود ندارد. نتیجه‌گیری: بر اساس یافته‌های این مطالعه، مواجهه مزمن با صداهای بالاتر از حد مجاز تأثیر معناداری بر تغییرات هورمون TSH در کارگران صنعت کاشی و سرامیک نداشته است. اگرچه ضرورت انجام مطالعات آتی با طراحی جامع‌تر و بررسی چندین شاخص هورمونی ضروری به نظر میرسد.   Background: Noise pollution is one of the most critical occupational hazards. One potential consequence of long-term noise exposure is alterations in endocrine function, particularly thyroid hormones. This study aimed to examine the trends of Thyroid-Stimulating Hormone (TSH) levels among ceramic and tile industry workers over a five-year period and its association with chronic noise exposure. Methods: This historical cohort study was conducted from 2016 to 2021 in a ceramic and tile plant. The participants included 100 workers from high-noise departments (exposure group) and 100 workers from departments with noise levels below the permissible limits (control group). Demographic data and TSH levels were extracted and recorded from medical records. Data were analyzed using independent t-tests and repeated measures ANOVA in SPSS version 24. Findings: The mean TSH levels in the baseline year (2016) were 2.93 ± 1.53 and 2.89 ± 1.36 in the exposure and control groups, respectively. Throughout the study period, changes in this hormone followed an irregular pattern in both groups, reaching 3.14 ± 1.59 and 3.10 ± 1.35 in 2021, respectively. Results from the independent t-test and repeated measures ANOVA indicated no significant difference in TSH changes between the two groups over time. Conclusion: Based on the findings of this study, chronic exposure to noise levels exceeding permissible limits did not have a significant impact on TSH level variations among ceramic and tile industry workers.
