زمینه و هدف: این مطالعه به بررسی علل رویدادهای احتمالی ناشی از خطاهای انسانی در عملیات های تعمیر و نگهداشت صنعت پتروشیمی به کمک HFACS می پردازد و هدف آن پیش بینی و شناسایی این علل و ارائه اقداماتی به منظور کاهش و پیشگیری از آنهاست.

روش بررسی: در این مطالعه اداره تعمیرات شرکت پتروشیمی زاگرس واقع در عسلویه جهت بررسی انتخاب شد. چک لیستی از علل بروز رویدادهای ناشی از خطای انسانی بر اساس سطوح 4 گانه و گروه های علتی 19 گانه در HFACS تهیه گردید. سپس به کمک تکنیک HTA فعالیت ها و وظایف کاری تعیین و با استفاده از چک لیست، علل 19 گانه در بروز خطاهای انسانی شناسایی و ثبت شد. سپس اولویت اقدامات اصلاحی و پیشگیرانه تعیین شد.

یافته ها: نتایج حاصل از تحلیل برگه های کار 444 فعالیت تعمیراتی نشان داد که در سطوح 4 گانه HFACS ، 37.6% از علل در سطح اعمال نا ایمن، 27.5% از علل در سطح نظارت و سرپرستی نا ایمن، 20.9% از علل در سطح پیش شرط های اعمال نا ایمن و 14% از علل در سطح تاثیرات سازمانی قرار دارند. در گروه های علتی نیز خطاها با 24.3% ، سرپرستی و نظارت ناکافی با 14.8% و تخلفات با 13.2% ، بیشترین میزان علل را به خود اختصاص می دهد.

نتیجه گیری: بر طبق یافته ها، افزایش اثربخشی آموزش کارکنان و بهبود نظارت بر عملکرد کارکنان به ترتیب بیشترین نقش را در کاهش وقوع رویدادهای ناشی از خطای انسانی در عملیات های تعمیر و نگهداشت پتروشیمی دارند.
